Facility reservation Request

Please note: Please fill in all required fields and submit 3 weeks prior to your requested date. The form will not be accepted if required fields are not completed and the appropriate time line is not followed.

Please note: By submitting this form you are requesting RWC Facility space, this does NOT confirm your reservation. You will be contacted within 3 business days through your email address with a response to your request. Submitting this form 3 weeks prior to your event date allows for a better chance for it to be approved.

You must use either Internet Explorer 6.0 or FireFox 1.5 to complete this form.

-Reservation time includes time to setup for your event. The event time should be the duration of the event, start time and end. Reservation end should be what time your event will be cleaned up and finished.
